PORTLAND, Maine (AP) — The Titan submersible disaster could have been prevented, the U.S. Coast Guard said in a report Tuesday (Aug. 5, 2025) that held OceanGate CEO Stockton Rush responsible for ignoring safety warnings, design flaws and crucial oversight which, had he survived, may have resulted in criminal charges.
Rush and four passengers were killed instantly deep below the North Atlantic in June 2023 when Titan suffered a catastrophic implosion as it descended to the wreck of the Titanic. A multiday search for survivors off Canada grabbed international headlines, and the tragedy led to lawsuits and calls for tighter regulation of the burgeoning private deep sea expedition industry.
The Coast Guard determined that safety procedures at OceanGate, a private company based in Washington state, were “critically flawed” and found “glaring disparities” between safety protocols and actual practices.
Preventing the next Titan disaster
Jason Neubauer of the Marine Board of Investigation said the findings will help avoid future tragedies.
“There is a need for stronger oversight and clear options for operators who are exploring new concepts outside of the existing regulatory framework,” he said in a statement.
Salvatore Mercogliano, a professor of maritime history at Campbell University in North Carolina, said OceanGate was able to exploit gray areas in maritime law that made it unclear who was responsible for enforcing regulations.
Now, he said, the Coast Guard and other international maritime agencies will be looking to improve coordination and implement clearer mechanisms for enforcement, especially for private submarines.
“Unfortunately almost all maritime regulation is written in blood, and something had to happen first,” Mercogliano said.
OceanGate suspended operations in July 2023. Spokesperson Christian Hammond said the company has been wound down and was fully cooperating with the investigation, and he offered condolences to the families of those who died and everyone affected.
‘Red flags’ at OceanGate
Investigators pointed to a culture at OceanGate of downplaying, ignoring and even falsifying key safety information to improve its reputation and evade scrutiny from regulators.
The company ignored “red flags” and had a “toxic workplace culture,” with firings of senior staff and the looming threat of dismissal used to dissuade employees and contractors from expressing safety concerns.
Rush, a former flight test engineer for fighter jets, founded the company in 2009 after years of experience in aerospace and aviation.
The Marine Board concluded that Rush had an “escalating disregard for established safety protocols,” which contributed to the deaths. If Rush were alive, the board would have passed the case to the U.S. Department of Justice and he may have faced criminal charges, it said.
Rush bragged that he would ‘buy a congressman’ if challenged
The company reclassified submersible passengers as “mission specialists” to bypass regulations on small passenger vessels and claim its subs were oceanic research vessels. Former mission specialists and OceanGate employees said their participation was “purely for a ride in the submersible, not for scientific research,” the report said.
Rush and OceanGate received numerous warnings about Titan’s fraudulent classifications. In 2017, Rush was told by a Coast Guard Reserve officer hired by OceanGate that his planned Titanic dive would be illegal.
Rush said “he would buy a congressman” if ever confronted by regulators, the officer testified.
Over the years the company resorted to increasingly deceptive strategies, the report said. By 2021 an OceanGate attorney falsely told a Virginia federal court, which was presiding over Titan’s authorization to conduct dives, that the vessel was registered in the Bahamas.
To obtain his credentials, Rush submitted a fraudulent sea service letter signed by OceanGate’s chief operations officer to the Coast Guard’s National Maritime Center, the report said. In it Rush claimed past service as a crew member on Titan and misrepresented the size of the vessel, when in fact it had never been registered or admeasured.
Titan’s inadequacies
Investigators found that the submersible’s design, certification, maintenance and inspection process were all inadequate. The vessel’s carbon fiber hull design and construction introduced flaws that “weakened the overall structural integrity” of the hull, according to the report.
Mounting financial pressures in 2023 led to a decision by OceanGate to store the submersible outdoors in the Canadian winter, and the hull was exposed to temperature fluctuations that also compromised the vessel’s integrity, the report said.
The victims
The implosion also killed French underwater explorer Paul-Henri Nargeolet, known as “Mr. Titanic”; British adventurer Hamish Harding; and two members of a prominent Pakistani family, Shahzada Dawood and his son Suleman Dawood.
Nargeolet’s family filed a $50 million lawsuit last year alleging that the crew experienced “terror and mental anguish” before the disaster. The lawsuit accused OceanGate of gross negligence.
Titan’s final dive
The vessel had been making voyages to the Titanic site since 2021. Its final dive came the morning of June 18, 2023. The submersible lost contact with its support vessel about two hours later and was reported overdue that afternoon. Ships, planes and equipment were rushed to the scene about 435 miles (700 kilometers) south of St. John’s, Newfoundland.
The Coast Guard-led team operated under the possibility that there could be survivors for several days. Wreckage was subsequently found on the ocean floor about 330 yards (300 meters) off the bow of the Titanic.

The disappearance of the Titan off Canada in 2023 led to a search that grabbed worldwide attention, and the Coast Guard convened its highest level of investigation in the aftermath.
The Titan was owned by OceanGate, a private company based in Washington state. The operator of the submersible, OceanGate head Stockton Rush, was among the five on board who died. There were no survivors.
The report found the company’s safety procedures were “critically flawed,” noting that the core of the failures inside the company came down to “glaring disparities” between their safety protocols and actual practices.
The submersible disaster has led to lawsuits and calls for tighter regulation of the developing private deep sea expedition industry.
Jason Neubauer, with the Marine Board of Investigation, said that the findings will help prevent future tragedies.

Spokespeople for OceanGate did not immediately respond to phone calls seeking comment on the report. OceanGate suspended operations in July 2023.
Investigators found that the submersible’s design, certification, maintenance and inspection process were all inadequate.
Throughout the report, which spans more than 300 pages, investigators repeatedly point to OceanGate’s culture of downplaying, ignoring and even falsifying key safety information to improve its reputation and evade scrutiny from regulators. OceanGate ignored “red flags” and had a “toxic workplace culture,” while its mission was hindered by lack of domestic and international framework for submersible operations, the report says.
Numerous OceanGate employees have come forward in the two years since the implosion to support those claims. The report says firings of senior staff members and the looming threat of being fired were used to dissuade employees and contractors from expressing safety concerns.
The report alleges that for several years preceding the Titan’s explosion, OceanGate “leveraged intimidation tactics, allowances for scientific operations, and the company’s favorable reputation to evade regulatory scrutiny.”
“By strategically creating and exploiting regulatory confusion and oversight challenges, OceanGate was ultimately able to operate TITAN completely outside of the established deep-sea protocols,” the report found.
The Marine Board said one challenge of the investigation was that “significant amounts” of video footage evidence that had been captured by witnesses was not subject to its subpoena authority because the witnesses weren’t U.S. citizens.
In addition to Rush, the implosion killed French explorer Paul-Henri Nargeolet, British adventurer Hamish Harding and two members of a prominent Pakistani family, Shahzada Dawood and his son Suleman Dawood.

Don Kramer also testified to the Coast Guard Wednesday (Sept. 25, 2024) that the material behaved differently after a loud bang was heard on one of the dives the year before the tragedy. OceanGate co-founder Stockton Rush was among the five people who died when the submersible imploded in June 2023. Titan’s unusual design subjected it to scrutiny in the undersea exploration community. Kramer said hull pieces recovered from the ocean floor after the tragedy showed substantial delamination of the layers of carbon fiber.

The investigatory panel has listened to three days of testimony that raised questions about the company’s operations before the doomed mission. UNDATED (AP)- A key employee who labeled a doomed experimental submersible unsafe prior to its last, fatal voyage testified Tuesday (Sept. 17, 2024) that he frequently clashed with the company’s co-founder and felt the company was committed only to making money.
David Lochridge, OceanGate’s former operations director, is one of the most anticipated witnesses to appear before a commission trying to determine what caused the Titan to implode en route to the wreckage of the Titanic last year, killing all five on board. His testimony echoed that of other former employees Monday, one of whom described OceanGate head Stockton Rush as volatile and difficult to work with.
“The whole idea behind the company was to make money,” Lochridge said. “There was very little in the way of science.”
Rush was among the five people who died in the implosion. OceanGate owned the Titan and brought it on several dives to the Titanic going back to 2021.
Lochridge’s testimony began a day after other witnesses painted a picture of a troubled company that was impatient to get its unconventionally designed craft into the water. The accident set off a worldwide debate about the future of private undersea exploration.
Lochridge joined the company in the mid-2010s as a veteran engineer and submersible pilot and said he quickly came to feel he was being used to lend the company scientific credibility. He said he felt the company was selling him as part of the project “for people to come up and pay money,” and that did not sit well with him.
“I was, I felt, a show pony,” he said. “I was made by the company to stand up there and do talks. It was difficult. I had to go up and do presentations. All of it.”
Lochridge referenced a 2018 report in which he raised safety issues about OceanGate operations. He said with all of the safety issues he saw “there was no way I was signing off on this.”
Asked whether he had confidence in the way the Titan was being built, he said: “No confidence whatsoever.”
Employee turnover was very high at the time, said Lochridge, and leadership dismissed his concerns because they were more focused on “bad engineering decisions” and a desire to get to the Titanic as quickly as possible and start making money. He eventually was fired after raising the safety concerns, he said.
“I didn’t want to lose my job. I wanted to do the Titanic. But to dive it safely. It was on my bucket list, too,” he said.
OceanGate, based in Washington state, suspended its operations after the implosion.
OceanGate’s former engineering director, Tony Nissen, kicked off Monday’s testimony, telling investigators he felt pressured to get the vessel ready to dive and refused to pilot it for a journey several years before Titan’s last trip. Nissen worked on a prototype hull that predated the Titanic expeditions.
“‘I’m not getting in it,’” Nissen said he told Rush.
When asked if there was pressure to get Titan into the water, Nissen responded, “100%.”
But asked if he felt that the pressure compromised safety decisions and testing, Nissen paused, then replied, “No. And that’s a difficult question to answer, because given infinite time and infinite budget, you could do infinite testing.”
OceanGate’s former finance and human resources director, Bonnie Carl, testified Monday that Lochridge had characterized the Titan as “unsafe.”
Coast Guard officials noted at the start of the hearing that the submersible had not been independently reviewed, as is standard practice. That and Titan’s unusual design subjected it to scrutiny in the undersea exploration community.
During the submersible’s final dive on June 18, 2023, the crew lost contact after an exchange of texts about the Titan’s depth and weight as it descended. The support ship Polar Prince then sent repeated messages asking if the Titan could still see the ship on its onboard display.
One of the last messages from Titan’s crew to Polar Prince before the submersible imploded stated, “all good here,” according to a visual re-creation presented earlier in the hearing.
When the submersible was reported overdue, rescuers rushed ships, planes and other equipment to an area about 435 miles (700 kilometers) south of St. John’s, Newfoundland. Wreckage of the Titan was subsequently found on the ocean floor about 330 yards (300 meters) off the bow of the Titanic, Coast Guard officials said.
Scheduled to appear later in the hearing are OceanGate co-founder Guillermo Sohnlein and former scientific director, Steven Ross, according to a list compiled by the Coast Guard. Numerous guard officials, scientists, and government and industry officials are also expected to testify. The U.S. Coast Guard subpoenaed witnesses who were not government employees, said Coast Guard spokesperson Melissa Leake.
Among those not on the hearing witness list is Rush’s widow, Wendy Rush, the company’s communications director. Asked about her absence, Leake said the Coast Guard does not comment on the reasons for not calling specific individuals to a particular hearing during ongoing investigations. She said it’s common for a Marine Board of Investigation to “hold multiple hearing sessions or conduct additional witness depositions for complex cases.”
OceanGate has no full-time employees at this time but will be represented by an attorney during the hearing, the company said in a statement. The company said it has been fully cooperating with the Coast Guard and NTSB investigations since they began.
The time frame for the investigation was initially a year, but the inquiry has taken longer. The ongoing Marine Board of Investigation is the highest level of marine casualty investigation conducted by the Coast Guard. When the hearing concludes, recommendations will be submitted to the Coast Guard’s commandant. The National Transportation Safety Board is also conducting an investigation.

PORTLAND, Maine (AP) — Human remains have likely been recovered from the wreckage of the submersible that imploded during an underwater voyage to view the Titanic, the U.S. Coast Guard said Wednesday (June 28, 2023).
The news came hours after the announcement that debris from the Titan, collected from the seafloor more than 12,000 feet (3,658 meters) below the surface of the North Atlantic, had arrived in St. John’s, Newfoundland. Twisted chunks of the submersible were unloaded at a Canadian Coast Guard pier.
Recovering and scrutinizing the wreckage is a key part of the investigation into why the Titan imploded last week, killing all five people on board. The multiday search and eventual recovery of debris from the 22-foot (6.7-meter) vessel captured the world’s attention.
“There is still a substantial amount of work to be done to understand the factors that led to the catastrophic loss of the Titan and help ensure a similar tragedy does not occur again,” Coast Guard Chief Capt. Jason Neubauer said in a statement released late Wednesday afternoon.
The “presumed human remains” will be brought to the United States, where medical professionals will conduct a formal analysis, Neubauer said. He added that the Coast Guard has convened an investigation of the implosion at the highest level. The Marine Board of Investigation will analyze and test evidence, including pieces of debris, at a port in the U.S. The board will share the evidence at a future public hearing whose date has not been determined, the Coast Guard said.
Neubauer said the evidence will provide “critical insights” into the cause of the implosion.
Debris from the Titan, which is believed to have imploded on June 18 as it made its descent, was located about 12,500 feet (3,810 meters) underwater and roughly 1,600 feet (488 meters) from the Titanic on the ocean floor. The Coast Guard is leading the investigation, in conjunction with several other government agencies in the U.S. and Canada.
Authorities have not disclosed details of the debris recovery, which could have followed several approaches, according to Carl Hartsfield, who directs a lab at the Woods Hole Oceanographic Institution that designs and operates autonomous underwater vehicles and has been serving as a consultant to the Coast Guard.
“If the pieces are small, you can collect them together and put them in a basket or some kind of collection device,” Hartsfield said Monday. Bigger pieces could be retrieved with a remote-operated vehicle, or ROV, such as the one brought to the wreckage site by the Canadian ship Horizon Arctic to search the ocean floor. For extremely big pieces, a heavy lift could be used to pull them up with a tow line, he said.
Representatives for Horizon Arctic did not respond to requests for comment. The ROV’s owner, Pelagic Research Services, a company with offices in Massachusetts and New York, is “still on mission” and cannot comment on the investigation, company spokesperson Jeff Mahoney said Wednesday.
“They have been working around the clock now for 10 days, through the physical and mental challenges of this operation,” Mahoney said.
Analyzing the recovered debris could reveal important clues about what happened to the Titan, and there could be electronic data recorded by the submersible’s instruments, Hartsfield said.
“So the question is, is there any data available? And I really don’t know the answer to that question,” he said Monday.
The Transportation Safety Board of Canada, which is conducting a safety investigation into the Titan’s Canadian-flagged mother ship, the Polar Prince, said Wednesday that it has sent that vessel’s voyage data recorder to a lab for analysis.
Stockton Rush, the Titan’s pilot and CEO of OceanGate Expeditions, the company that owned the submersible, was killed in the implosion along with two members of a prominent Pakistani family, Shahzada Dawood and his son Suleman Dawood; British adventurer Hamish Harding; and Titanic expert Paul-Henri Nargeolet.
OceanGate is based in the U.S. but the submersible was registered in the Bahamas.
The company charged passengers $250,000 each to participate in the voyage. The implosion of the Titan has raised questions about the safety of private undersea exploration operations. The Coast Guard wants to use the investigation to improve the safety of submersibles.

Experts say the ocean floor is a “noisy” place and a potential connection between the banging and the Titan were unlikely. Debris from the Titan, which suffered a catastrophic implosion that killed all five men aboard, was found near the Titanic shipwreck deep in the North Atlantic waters.

UNDATED (AP)- The pilot and four passengers of a missing submersible that disappeared en route to the wreckage of the Titanic are believed to be dead, the expedition company said Thursday (June 22, 2023).
Pilot and chief executive Stockton Rush, along with passengers Shahzada Dawood and his son Suleman Dawood, Hamish Harding, and Paul-Henri Nargeolet “have sadly been lost,” OceanGate Expeditions said in a statement.
OceanGate did not provide details when the company announced the “loss of life” in a statement or how officials knew the crew members perished. The Titan’s 96-hour oxygen supply likely ended early Thursday.
OceanGate has been chronicling the Titanic’s decay and the underwater ecosystem around it via yearly voyages since 2021.
Later Thursday, U.S. Coast Guard officials said the search had uncovered parts of the Titan consistent with an implosion. The Coast Guard said the pieces were discovered within the search area by a remotely operated underwater robot.
The Titan was estimated to have about a four-day supply of breathable air when it launched Sunday morning in the North Atlantic — but experts have emphasized that was an imprecise approximation to begin with and could be extended if passengers have taken measures to conserve breathable air. And it’s not known if they survived since the sub’s disappearance.
Rescuers have rushed ships, planes and other equipment to the site of the disappearance. On Thursday, the U.S. Coast Guard said an undersea robot sent by a Canadian ship had reached the sea floor, while a French research institute said a deep-diving robot with cameras, lights and arms also joined the operation.
Authorities have been hoping underwater sounds might help narrow their search, whose coverage area has been expanded to thousands of miles — twice the size of Connecticut and in waters 2 1/2 miles (4 kilometers) deep. Coast Guard officials said underwater noises were detected in the search area Tuesday and Wednesday.
Jamie Pringle, an expert in Forensic Geosciences at Keele University, in England, said even if the noises came from the submersible, “The lack of oxygen is key now; even if they find it, they still need to get to the surface and unbolt it.”
The Titan was reported overdue Sunday afternoon about 435 miles (700 kilometers) south of St. John’s, Newfoundland, as it was on its way to where the iconic ocean liner sank more than a century ago. OceanGate Expeditions, which is leading the trip, has been chronicling the Titanic’s decay and the underwater ecosystem around it via yearly voyages since 2021.
By Thursday morning, hope was running out that anyone on board the vessel would be found alive.
Dr. Rob Larter, a marine geophysicist with the British Antarctic Survey, emphasized the difficulty of finding something the size of the submersible, which is about 22 feet (6.5 meters) long and 9 feet (nearly 3 meters) high.
“You’re talking about totally dark environments,” in which an object several dozen feet away can be missed, he said. “It’s just a needle in a haystack situation unless you’ve got a pretty precise location.”
Newly uncovered allegations suggest there had been significant warnings made about vessel safety during the submersible’s development.
Broadcasters around the world started newscasts at the critical hour Thursday with news of the submersible. The Saudi-owned satellite channel Al Arabiya showed a clock on air counting down to their estimate of when the air could potentially run out.
Captain Jamie Frederick of the First Coast Guard District said a day earlier that authorities were still holding out hope of saving the five passengers onboard.
“This is a search-and-rescue mission, 100%,” he said Wednesday.
Retired Navy Capt. Carl Hartsfield, now the director of the Woods Hole Oceanographic Systems Laboratory, said the sounds detected have been described as “banging noises,” but he warned that search crews “have to put the whole picture together in context and they have to eliminate potential manmade sources other than the Titan.” Frederick acknowledged Wednesday that authorities didn’t what the sounds were.
The report of sounds was encouraging to some experts because submarine crews unable to communicate with the surface are taught to bang on their submersible’s hull to be detected by sonar.
The U.S. Navy said in a statement Wednesday that it was sending a specialized salvage system that’s capable of hoisting “large, bulky and heavy undersea objects such as aircraft or small vessels.”
The Titan weighs 20,000 pounds (9,000 kilograms). The U.S. Navy’s Flyaway Deep Ocean Salvage System is designed to lift up to 60,000 pounds (27,200 kilograms), the Navy said on its website.
Lost aboard the vessel is pilot Stockton Rush, the CEO of OceanGate. His passengers are: British adventurer Hamish Harding; Pakistani businessman Shahzada Dawood and his son Suleman; and French explorer and Titanic expert Paul-Henry Nargeolet.
In the first comments from Pakistan since the Titan vanished, Pakistani Foreign Ministry spokesperson Mumtaz Zahra Baloch said Thursday that officials have confidence in the search efforts.
“We would not like to speculate on the circumstances of this incident and we would also like to respect the wishes of the Dawood family that their privacy be respected,” she said.
At least 46 people successfully traveled on OceanGate’s submersible to the Titanic wreck site in 2021 and 2022, according to letters the company filed with a U.S. District Court in Norfolk, Virginia, that oversees matters involving the Titanic shipwreck.
One of the company’s first customers characterized a dive he made to the site two years ago as a “kamikaze operation.”
“Imagine a metal tube a few meters long with a sheet of metal for a floor. You can’t stand. You can’t kneel. Everyone is sitting close to or on top of each other,” said Arthur Loibl, a retired businessman and adventurer from Germany. “You can’t be claustrophobic.”
During the 2 1/2-hour descent and ascent, the lights were turned off to conserve energy, he said, with the only illumination coming from a fluorescent glow stick.
The dive was repeatedly delayed to fix a problem with the battery and the balancing weights. In total, the voyage took 10 1/2 hours.
The submersible had seven backup systems to return to the surface, including sandbags and lead pipes that drop off and an inflatable balloon.
Nick Rotker, who leads underwater research for the nonprofit research and development company MITRE, said the difficulty in searching for the Titan has underscored the U.S.’s need for more underwater robots and remotely operated underwater vehicles.
“The issue is, we don’t have a lot of capability or systems that can go to the depth this vessel was going to,” Rotker said.
Nicolai Roterman, a deep-sea ecologist and lecturer in marine biology at the University of Portsmouth, England, said the disappearance of the Titan highlights the dangers and unknowns of deep-sea tourism.
